Your first visit to Ayr Dental involves a completion of new patient registration/medical history forms. For your convenience you can download it below and complete it at the comfort of your own home. During your first appointment, we will complete a comprehensive oral exam, take x-rays and discuss the best treatment options with you.
If you are moving from another dental office, let us know so that we can request the transfer of your patient records.
If you have dental insurance, feel free to bring your information to the first appointment. We will bill your insurance directly and eliminate any extra paper work for you. We also accept all methods of payment including checks, cash, debit, or credit cards.
